Drug Reorder Book

For use with medication/blister cards in the nursing home, our drug reorder books are available with, or without, a reorder tab. Each page features an order log of dispensed medications. Our drug reorder book with reorder tab has a removable section featuring the Rx number, patient name, and drug name which can be conveniently forwarded to the pharmacy as needed.

Contolled Substances Register

Each controlled substance register book maintains a permanent record for accurately recording the usage of controlled narcotics. The book measures 10" x 12" and contains 270 pages.

Rx Blanks

We have 4 1/8" x 5 1/4" custom imprinted and stock prescription blanks available.  Our MA-1 style blank is printed in blue ink on white bond paper. The minimum order for stock prescription blanks is 1,000 (10 pads) and the minimum order for custom imprinting is 4,000 Rx blanks (40 pads of 100 sheets per pad).

Physician's Orders/M.A.R Form

Keep track of any changes in medication with this handy all-in-one notebook-style form. Part 1 (chart copy) and Part 2 (pharmacy copy) are carbonless and contain duplicate information. Part 3 (M.A.R) is completed during the last week of a month in anticipation for the next month. Minimum order requirement is 1 box containing 600, 3-part forms.
